5e Planescape: Tales of Devotion
Soul Gem of Clemence Patafian
The glimmer of this milky oval moonstone almost seems curious about its surroundings.
Wondrous Item, legendary (requires attunement)
This oval moonstone the size of a human eyeball contains the magically trapped soul of Princess Clemence Patafian von Schilda, the third-born daughter of Grand Duke Gumbu Patafian von Schilda of the grand city of Schilderheim. She was released from her sickly aasimar body by the cambion Sitri the Sly in order for him to show her the marvels of the planes.
In order to be used, the soul gem must be implanted on either the user's forehead or chest. This melding is achieved magically by attuning with the item and it is painless.
The soul gem has the following properties:
Charisma Bonus. The user's Charisma score is 15 while they have this gem implanted. It has no effect on the user if their Charisma is 15 or higher without it.
Darkvision. The user can see in dim light within 60 feet of them as if it were bright light., and in darkness as if it were dim light. They cannot discern color in darkness, only shades of grey.
Celestial Resistance. The user has resistance to necrotic damage and radiant damage.
Self-healing. The soul gem can restore a total number of hit points equal to the user’s Charisma modifier x5. This pool refreshes at a rate of 2d8 per long rest. As an action, the user can focus on the gem and draw power from this pool to restore a number of hit points, up to the maximum amount remaining in the pool. Alternatively, they can expend 5 hit points from the pool of healing to cure themselves of one disease or to neutralize one poison affecting them. The user can cure multiple diseases and neutralize multiple poisons with a single use of this ability.
Sentience. The Soul Gem of Clemence Patafian houses the sentience of Princess Clemence. She is a lawful good aasimar with an Intelligence of 12, a Wisdom of 15m and a Charisma of 15. The gem has hearing and darkvision out to a range of 60 feet.
The gem can read and understand Celestial and Common, and can communicate with its user telepathically.
Personality. The gem houses the personality of Princess Clemence. She is an assertive yet gentle young woman who loves to converse and debate about ethics, geography, and anthropology in length.
The purpose of the gem is to allow Princess Clemence to persist beyond the limits of her failing body and to allow her to see as much of existence in all its forms as possible. In particular, the cambion Sitri the Sly promised to show the princess the planes in all their glory. If the user shows no signs of wanting to indulge the spirit of the gem with conversation about her preferred topics or tries to settle in one place for too long, a conflict between them and the gem occurs at the next sunrise.
